Heart failure patients management has a huge impact on health systems due to the required multiple and prolonged hospitalisations of patients and the related patient management costs. Focusing on routine patient-physician or patient-nurse communication following discharge can result in a significant reduction of hospitalizations.

Introducing daily patient monitoring of Heart Failure patients outside hospitals in order to collect different types of clinical, behavioural and real-world data for them.
Analysing the collected data, using novel data analytics and artificial intelligence technologies, with the aim to identify patterns and associations in the data that can improve the clinical management of patients, reduce hospitalisations and death rate and improve their quality of life.
Assessing the potential of using remote Heart Failure Patients monitoring, , including patient safety and quality and the necessity for remote monitoring and support for a continually increasing older adult population living with complex, multimorbid conditions and presenting health services with an ever-increasing strenuous demand.
RETENTION is an innovative platform supporting enhanced clinical monitoring and interventions aimed at improving the clinical management of patients with chronic Heart Failure, reducing their mortality and hospitalisation rates, and improving their quality of life, safety, and well-being.

The platform supports clinical decision making and evidenced based personalised interventions for HF patients by:
Data analytics using innovative model-driven big data methods, statistical approaches, artificial intelligence and machine learning techniques.
Applying transparent, explainable and verifiable decision making capabilities that leverage the evidence produced by the underlying data analysis and augment clinical studies targeting HF and, potentially, other CVDs.
CLINICAL SITE BACKEND
Patterns detection in the HF disease progression and the quality of life of patients.
Cross checking and validation against the clinical literature.
